1095-C Tax Information

According to new Federal requirements, you may receive a copy of your Form 1095-C upon request. This means that Harnett County Schools is no longer required to automatically send you a 1095-C. 

If you would like a copy of your Form 1095-C, click here to access our benefits portal at the State Health Plan eBenefits website to download your Form 1095-C electronically.  After logging in, hover over the “Benefits” tab at the top right and then select “1095 Tax Documents” from that menu to download the document

If you are no longer employed by Harnett County Schools or you retired in 2025, you will receive your Form 1095-C in the mail.
