Student-Athlete Registration & Forms
All students participating in athletics at Harnett County Schools must complete the registration process through Aktivate before participating in practices or competitions.
Step 1:
Parents/guardians must create an account and complete the required student-athlete registration forms through Aktivate.
Most required athletic forms are completed electronically through the Aktivate platform.
Step 2:
The NCHSAA requires all student-atheltes to have a current physical examination on file.

Once completed and signed by a healthcare provider, the physical form can be uploaded directly to Aktivate. Parents may upload a scanned copy from a computer or take photos of the completed form using a mobile device.
