What Services are Provided

  • Mentoring

  • Advocacy

  • Home visits

  • Collaborate with other professionals (e.g., nurses, county mediator, court counselors, therapists, etc.)

  • Link students and families to community resources  (e.g., food banks, social services, hospitals, Department of Juvenile Justice, etc.)

How Services are Initiated

School staff, parents, community members and students can submit referrals verbally or in writing.  The referrals can be delivered directly to the coordinators or they can be made by contacting the student's  local school guidance office.  Referrals are processed and contact is established as soon as possible.  A plan for academic success is designed for the student and the coordinators provide needed support.
